HHC staff members certified as CHOICES counselors

center-for-healthy-agingTwo staff members of Hartford HealthCare Center for Healthy Aging recently became certified as CHOICES counselors to guide individuals in navigating the Medicare process.

Jessica Dakin, BSN, CMC, geriatric care manager, and Marc Levesque, MS, senior resource case manager, achieved the certification following five days of Medicare training and passing a certification exam.

CHOICES counselors advise, educate and empower persons in understanding the increasingly complex Medicare program, helping beneficiaries make choices among a vast array of options to best meet their needs. Hartford HealthCare Center for Healthy Aging has been offering confidential, one-on-one sessions with individuals needing help making the optimum decisions. During a recent session, a CHOICES counselor succeeded in saving a client thousands of dollars in costs.

Through the Hartford HealthCare Center for Healthy Aging, Dakin provides comprehensive medical, physical and social needs assessments and develops, coordinates and modifies individual care plans to assist with day-to-day activities for her patients. She arranges and monitors in-home help or services, acts as a liaison to families who are at a distance, and attends medical appointments. She provides geriatric care manager services for a private pay fee.

Levesque, a recipient of the Connecticut Hospital Association Healthcare Hero Award, conducts comprehensive home, office and telephonic assessments. He uses his wealth of knowledge to provide information and access to community resources needed by seniors and their caregivers to enhance their quality of life.

On Nov. 4, Dakin facilitated a public workshop at the Bradley campus, but there is still limited availability on Friday, Dec. 2, from 9 a.m. to 4 p.m., at MidState Medical Center, 435 Lewis Ave., Meriden, Conference Center V. Levesque will facilitate.
