Martinson named as program director at Bradley

Wendy Martinson, program director of the Connecticut Center for Health Aging, recently announced the hiring of Michelle L. Lavoie as the newest resource coordinator to assist seniors, families, and caregivers in accessing the essential services to successfully age in place.

Lavoie is based at the Connecticut Center for Healthy Aging office at The Hospital of Central Connecticut, Bradley Memorial campus, located at 81 Meriden Ave. She began this position on Aug. 15.

Lavoie, a Bristol resident, was most recently employed as a care transitions coach with Connecticut Community Care to prepare individuals in managing their health care needs upon discharge from the hospital to reduce readmissions. Her professional experience also includes serving as a care manager, residential instructor for people with developmental disabilities and employment specialist. She holds a Bachelor of Arts degree in psychology from Springfield College, in Springfield, Massachusetts.

As resource coordinator with the Connecticut Center for Healthy Aging, a Hartford HealthCare partner, Lavoie provides home, office or telephone assessments and information to seniors and families about available resources to improve the quality of life. This service streamlines the process of coordinating necessary programs and interventions including state programs, home care, medical alert services and home modifications such as the installation of a handicap ramp.

Lavoie and the other Connecticut Center for Healthy Aging resource coordinators work collaboratively across the system to share the most current information to provide the best resources.

“It is rewarding to have a position that allows me to touch people’s lives and let them know that someone cares and is there to provide assistance and support,” Lavoie said in a Connecticut Center for Health Aging press release.

This new position is one of several positions funded by the $2.1 million grant the Connecticut Center for Healthy Aging received late last year from the state Department of Social Services.

The Connecticut Center for Healthy Aging, a not for profit member of Hartford HealthCare Senior Services, is a resource and assessment center designed to enhance access to services and information related to attaining optimal quality of life for seniors and their caregivers.
